Medical Forms (registered patients only)

 

Please DO NOT book an appointment to request this service. Read the instructions below on how to proceed:

Step 1. Identify if having your form completed will be covered by a 3rd party or if you are responsible for payment. Please see the list of prices for the most common forms below:

Common Uninsured forms:

  1. Disability Tax Credit Certificate, Form T2201 starting at $180 - $236

  2. Driver’s Medical Form $100

  3. Medical Certificates/Doctor’s notes starting at $30

  4. Parking Permit Application $30

  5. Other Simple Medical Forms or Certificates / EI Form starting at $52.90

  6. Other Moderate Complexity Medical Forms starting at $90

  7. Other Complex Medical Forms, starting from $236

Common Insured forms:

  1. Medical Employability Form

  2. Canada Pension Plan Disability Benefit Application ISP2519

  3. Persons with Disability (PWD) Designation Application Form (3 part booklet)

  4. ICBC Medical Report

 

Step 2. If the form that you require completed is listed above, please go to Step 3. 

If the form that you require completed is not listed above, please attach it on a message on your Patient Portal Account or drop it off at the clinic so it can be reviewed and priced based on the form complexity. If the clinic is closed, you can slide it under the door of Suite 300 in an envelope addressed to Dr. Lina Suazo. Ensure you have completed all your sections.

Once the form is reviewed, you will be informed of the fee and will be requested to pre pay (if not covered by a 3rd party). Your doctor may or may not need that you follow-up with an office visit; you will also be notified of this. Now go to Step 4.

Step 3. After identifying your medical form, answer and sign all sections that you are responsible to complete and bring it to the office. IMPORTANT: after the doctor reviews your Form, you may be asked to book an appointment.

Step 4. Proceed to pay for your Form completion. You can go to payment options below. All form completions are to be paid Up Front.

Step 5. You will be notified when your form has been sent in or ready for you to pick-up.  

Note: Forms may take up to 2 weeks to be completed. Your doctor is mainly focused on providing medical care for her patients during business hours these requests are attended to after hours.
